Weekend 1
The Annual Event
January · Kerrville, TX
Join the full Crown Council community at the Annual Event. The following day, spend dedicated time with Dr. Hornbrook on smile design, occlusion, prep design, cementation, and materials — anchored to your patient's case. The charity recipient is selected from the cohort.
Weekend 2
Preparation
Summer · Salt Lake City
The recipient travels to Salt Lake City. You work alongside Dr. Hornbrook and the cohort on the preparation phase — diagnostic, treatment planning, and clinical preparation. Hands-on dentistry, not a lecture.
Weekend 3
Seating
Summer · Salt Lake City
The final delivery. You see the restoration seated, the patient transformed, and you leave with a framework you can use immediately. Then you graduate into the Crown Council — a 1,000-practice alliance for life.
The Charitable Case

Each Young Dentist Program participant nominates one of their own patients for the charitable smile makeover. The cohort selects the most deserving case — and the patient receives a complete cosmetic transformation at no cost.
Save-a-Smile / Save-a-Life is one of the longest-running charitable initiatives in dentistry. Every participant leaves with a working playbook for diagnosis, materials, treatment planning, and case acceptance — built on a real patient with real stakes.
This is the part of the program that makes the work feel like vocation, not a job. It's also the part Crown Council members say transforms their team's culture more than any other single experience.

I'm brand-new in practice. Is the Young Dentist Program for me? +
Yes. The program was designed specifically for dentists 1 to 5 years out of dental school. New graduates often get the most out of it because the foundational gaps — case presentation, cosmetic case selection, team culture, treatment planning for higher-value cases — are exactly what the curriculum is built around.
If you graduated more than 5 years ago, our Forever Young alumni program is a better fit.
What does the $2,400 actually cover? +
Admission to all three mentored weekend events, 48 hours of accredited CE, admission to the Crown Council Annual Event, your 1-year Crown Council full membership, access to Dr. Hornbrook's smile design template library, alumni community access, and monthly office hours for the full year.
Your own travel, hotel, and food when you attend the Annual Event and the two Salt Lake City weekends are your responsibility. The Crown Council covers travel, hotel, and food for the charity patient. The laboratory restoration is donated by Utah Valley Dental Lab.
How is this different from other cosmetic CE programs? +
Most cosmetic continuum programs run $10,000 to $40,000 and are designed for established dentists with years of clinical experience and the disposable income to absorb a major continuum investment.
The Young Dentist Program is built for the first 5 years of your career. It's a 12-month accelerator, not a multi-year continuum. It includes a live patient case (which is rare in this space). And it's priced so being early in your career — and carrying student debt — isn't a reason to wait.
Many of our graduates go on to advanced continuum programs later. The Young Dentist Program is the on-ramp, not the destination.
What if I can't find a patient to nominate? +
Most participants find a patient within their first 30 days. We provide consult scripts and patient screening guides specifically for identifying the right candidate. Even if your own patient is not selected as the official charity case, you participate in the case work, the treatment planning, and the clinical execution as part of the group.
How do you select the charitable patient? +
Participants present nominated patients during the January Annual Event. The full Young Dentist Program cohort discusses the cases and selects the recipient based on clinical suitability, story, and impact. The patient must have good periodontal health, be able to tolerate the clinical work, and be willing to travel to Salt Lake City.
What happens after the year ends? +
Most graduates continue as full Crown Council members at $129/month — staying inside the 1,000-practice alliance, attending future Annual Events, and accessing the ongoing education library. Some go on to advanced continuums for further clinical depth. Either way, your relationship with the alumni network and the Crown Council community continues for as long as you want it to.
When do applications close? +
Applications for the 2027 program close December 31, 2026. Seats are filled on a first-come, first-served basis. The program has reached capacity each year, typically several weeks before the deadline.

Founded in 1996, Crown Council is an international alliance of more than 1,000 leading dental practices committed to building a Culture of Success in their offices and serving their communities through charitable work. For nearly three decades, the Smiles for Life Foundation — the charitable arm of Crown Council — has raised millions of dollars for children's charities through dental practices participating in annual whitening campaigns.
